2. The specifications of the bound book are uniformly made of A4 paper with a copper sheet on the cover; the name of the cover: the horizontal row at the top is "Safety production license for Construction Enterprises" (with No. 30 small mark Song); the middle vertical row is "declaration material" (with No. 48 boldface); the horizontal arrangement below is the full name of the declaration enterprise (with Book 22 Song), and the year, month and day (with Book 22 Song).
3. When the enterprise applies, the original documents and vouchers involved in the application materials (excluding the safety production examination certificate of the "three categories of personnel" of the enterprise) shall provide the original for verification.
4. The name of the working unit of the certificate in all the declaration materials shall be consistent with that of the declaration unit.

Enterprises that have obtained a production safety license and are within the validity period (those that need to be extended will apply for extension, and they need to be entered and filed online. When entering, they only need to submit the following scanned copies:

3. Organization code certificate;
4. Safety production license;
5. Certificate of safety production assessment for three types of personnel.
